This is a unique opportunity to lead chronic disease and renal health initiatives in a culturally rich, community-controlled environment in the Katherine region. Job Details Base salary commensurate with experience 6 weeks annual leave 18% leave loading Salary packaging options up to $15,996 per FBT year $2,650 for meals & entertainment Relocation assistance available Monday to Friday role, no shift work required Fully subsidised, modern family-friendly accommodation FOIL (Fly Out of Isolation Leave) available The Role Seeking an experienced clinician and manager with expertise in chronic condition care and a commitment to improving health outcomes for Aboriginal people through coordinated, culturally appropriate services. This position plays a critical leadership role in improving outcomes for Aboriginal people living with or at risk of chronic disease. Key Responsibilities Provide strategic and operational leadership for chronic disease programs Embed renal health planning through the TKC framework Supervise and support a multidisciplinary team delivering coordinated care Strengthen quality improvement systems and ensure compliance Facilitate visiting specialist services and allied health integration Lead internal reporting, performance tracking, and data-informed decision making Represent Sunrise in governance, sectoral, and clinical advisory forums To be eligible you must: Hold current AHPRA registration as a Registered Nurse, Allied Health Professional, or equivalent Have a minimum of 5 years’ experience in chronic disease management and/or regional program delivery Demonstrate a strong understanding of chronic conditions and social determinants of health in Aboriginal contexts Possess leadership experience in a remote health setting Be culturally competent, adaptable, and an effective communicator Hold a National Police Check (or ability to obtain) Hold current OCHRE card (or ability to obtain) Meet NT Health vaccination requirements Have full Australian working rights Opportunities in the Northern Territory are in demand, send you cv through today and we will be in touch immediately!
