About the Role: As a key member of our multidisciplinary team, you will provide high-quality, person-centred support across acute, subacute, residential aged care, and community programs. You’ll play a vital role in: Providing psychosocial assessments and interventions Supporting patients through transitions of care and discharge planning Advocating for client needs and linking with community services Working closely with patients, families, carers, and healthcare teams What We’re Looking For: Demonstrated experience in a healthcare or community setting Strong communication, advocacy, and interpersonal skills A commitment to collaborative, culturally sensitive, and trauma-informed care Why Yarrawonga Health?
