As we grow, we're looking for a compassionate and organised Care Partner to join our team — someone who’s not only client-focused, but also confident managing rosters and coordinating support across Self-Managed NDIS and Home Care Packages (HCP). Your Role as Care Partner: As a Care Partner, you’ll work closely with clients, families, and support workers to ensure care is delivered with empathy, accuracy, and heart. This is a hands-on role combining client engagement, care coordination, and rostering responsibilities — ideal for someone who thrives in a fast-paced, people-first environment. What You'll Be Doing: Support clients in navigating their NDIS (self-managed) or Home Care Package (HCP) funding Build trusting relationships with clients, families, and care workers Manage and coordinate care services to meet individual client needs and preferences Create and maintain effective, client-centred rosters for support workers Respond to enquiries, concerns, and changes in client circumstances promptly and with care Collaborate with internal teams and external providers to ensure smooth service delivery Maintain accurate documentation and ensure compliance with sector regulations What We’re Looking For: Experience in care coordination, rostering or a similar client-facing role Knowledge of the aged care and/or NDIS sectors (especially self-managed & HCP) Excellent time management and communication skills A can-do attitude with a strong focus on client care and outcomes Computer literacy (especially rostering systems or CRMs) Police Check, WWCC, NDIS Worker Screening Check (or willing to obtain) Current First Aid & CPR (or willingness to obtain) Minimum 2 doses of COVID-19 vaccination Why Work With Us?
