Physiotherapist – Community Home Care Stream Healthcare are seeking a passionate Community Physiotherapist to deliver high-quality, in-home care to clients of all ages across South Brisbane and Logan and the surrounds. Caseloads are funded through Home Care Packages, NDIS, or privately managed. Key Responsibilities: Conduct comprehensive in-home assessments and develop personalised treatment plans Set SMART goals and educate clients and families for long-term health outcomes Collaborate with multidisciplinary teams Maintain clear, timely documentation and reports See up to 8 clients per day (initial assessments and follow-ups) Participate in team meetings and professional development Looking for a flexible, proactive physio ready to thrive in a dynamic, client-focused environment. Full Drivers Licence Own Vehicle fully insured and road worthy Bachelor/Masters or Doctorate of Physiotherapy Current Police Check Clearance Working with Children Check Full or Limited Registration with Australian Heath Practitioner Regulatory Authority (AHPRA) Please note: NO sponsorship can be provided for this position.
