As part of the Check-In Team at Frontyard, you'll play a pivotal role in delivering efficient, effective, and hands-on therapeutic services, with plenty of opportunities to connect and engage in fun activities with young people. HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients. Operationally the role is based at Frontyard Youth Services which is a specialist youth service located in Melbourne’s CBD. Frontyard aims to support young people aged 12 to 24 to meet their physical, emotional and social needs and to develop pathways out of homelessness. This includes but is not limited to providing 18 beds crisis accommodation service and drop in centre housing multiple services working with young people across Greater Melbourne and throughout Victoria. 4. POSITION PURPOSE The primary responsibility of the role is to provide clinical care as part of the Check in Team based at Frontyard Youth Services ensuring delivery of an efficient, effective and clinically therapeutic service. As a care coordinator, you will ensure delivery of complex holistic health assessments and the planning for person centred client driven goals focusing on care co-ordination and improving self-management. This involves direct and indirect clinical care, maintaining an active caseload and supporting clients to develop self-management skills in relation to their complex care needs POSITION DUTIES Ensure delivery of an effective and responsive service for clients of Frontyard Actively screen and engage with clients within the accommodation service and drop-in centre. Manages a case load of clients in addition to providing support and brief interventions opportunistically with residents/clients, who may have limited service engagement Effective negotiation and advocacy with relevant service providers to ensure optimal and coordinated client care according to best practice principles Maintain effective communication and foster positive relationships with all internal and external stakeholders Undertake holistic comprehensive assessments with the client using motivational interviewing and self-management approaches. Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs. Facilitation of client generated goals that are realistic, time limited and based on addressing health and wellbeing needs. Support client referral and engagement with specialist clinical services as indicated by individual need Identification and Assessment of risk issues, including escalation to senior clinical and program staff, and timely documentation of the plan. Monitor health outcomes and plan for the transition and exit from the program, ensuring appropriate long term community supports are in place on discharge. Provide training and support in areas of specific expertise as needed Ensure all documentation, clinical handover and observations are completed each shift and in line with required SVHM and program standards Complete annual clinical competencies. 6. Highly developed interpersonal skills incorporating excellent verbal and written organisational and negation skills Demonstrated sound knowledge of the application of care coordination practice frameworks and principles, in the context of the current health care environment.
