About your opportunity: The position is based at both Cairns North and Gordonvale Hospital and primarily provides services to clients in that geographical area, however; there is an expectation that the position will travel within the Hinterland Hospital Service as required to provide a service to Transition Care clients across Cairns and Hinterland Hospital Health Service. About your role: Provide comprehensive education on a defined range of topics to patients or groups of patients. Work across Residential and Community settings of Transition Care Program. Lead a defined range of group interventions for patients with diverse and complex needs. Actively contribute as a member of a multidisciplinary team, including where appropriate, leading departmental and team meetings, case conferences and other team projects and activities.
