About the Role: We are seeking compassionate and dedicated Home Care Support Workers to join our team and provide exceptional care and assistance to individuals living with mental health and physical disabilities. In this role, you will be responsible for supporting participants with their activities of daily living (ADLs) and community access, ensuring they lead fulfilling, independent, and enriched lives. You will work closely with individuals to assist them in all aspects of their daily routines, from personal care to engaging in social and community activities, with a focus on maximising independence and promoting choice and control. Key Responsibilities: Assist participants with activities of daily living (ADLs), including personal care, mobility support, and medication management. Support individuals in community access activities, including attending appointments, social events, and engaging in recreational outings. Foster independence by encouraging skill development in daily tasks. Provide emotional and social support, promoting participant empowerment and well-being. Maintain accurate documentation, including shift notes, incident reports, and other documentation as required. Collaborate with other team members, to ensure holistic and quality care. Adhere to all NDIS standards, policies, and procedures to ensure quality and safety in service delivery. Approximate Shift Times: Morning Shifts – 7am to 3 pm Afternoon Shifts - 3pm to 11pm Active Overnights - 11pm to 7am Sleepover Shifts – 3pm to 7am Key Requirements: Previous experience in disability support or a related field is highly desirable. Certificate III or IV in Disability, Individual Support, or similar qualification (or willingness to obtain) Ability to work effectively both independently and as part of a team. Strong communication and interpersonal skills, with a compassionate and person-centered approach Valid driver’s license and reliable vehicle (may be required to assist participants with transportation) Current First Aid and CPR certificates (or willingness to obtain) Paid NDIS Worker Screening Card (or willingness to obtain) Why Join Us?
