Location: Woollahra Employment Type: Full-Time About the Role Anglicare is seeking a compassionate and proactive Health & Wellbeing Coordinator to support residents across our Retirement Villages. In this vital role, you will help residents maintain their independence by coordinating health, wellbeing, and social support services. Acting as a key point of contact for residents and their families, you will deliver exceptional customer service, support smooth transitions into care services, and promote health and wellness through assessments, referrals, and village-based programs. Reporting to the Village Manager you will build trusted relationships, advocate for residents’ needs, and contribute to vibrant community life within our villages. Key Responsibilities Deliver exceptional customer service and support residents’ health and wellbeing needs. Conduct assessments, develop personalised plans, and coordinate referrals through the My Aged Care process. Organise and promote activities that encourage social engagement and wellness. Maintain accurate records and ensure compliance with policies and aged care standards. Build positive relationships with residents, families, staff, and health professionals. What We’re Looking For Certificate IV or higher in Social Work, Social Sciences, Health Management, or related field. Experience in aged care or human services. Strong communication , organisation, and relationship-building skills. Current driver’s licence; First Aid Certificate preferred.
