About the opportunity Mercy Palliative Care is a 24 hour community-based palliative care service providing in home care to patients of all ages with life limiting illnesses. Our services covers the North and West Metropolitan region of Melbourne which incorporates seven municipalities including: Brimbank, Maribyrnong, Melbourne, Melton, Moonee Valley, Hobson Bay and Wyndham. Our multi-disciplinary team including Doctors, Nurses and Allied Health is committed to providing holistic care to patients and their family ensuring they experience the best quality of life while addressing their physical, emotional, social and spiritual needs. Our aim is to deliver quality patient care with focus on symptom management while providing support to patient and their families to relieve the psychological, financial and emotional burden during the palliative phase. We are looking for a committed Clinical Nurse Consultant to support our growing population and the the staff that provide care to them As a clinical leader you will: Be a clinical expert and resource, ensuring the delivery of best practice and evidence based care to our patient. Lead our Multidisciplinary team to ensure the delivery of care with compassion to people living with a terminal condition and dying as well as supporting carers. Provide clinical leadership and be a professional role model while practising within the core values of Mercy Health. Demonstrate a comprehensive understanding of the underpinnings of palliative care and a commitment to the delivery of timely and appropriate holistic care. Post graduate qualifications in palliative care or working towards required. What you’ll bring You’re resilient, compassionate and have a whole lot of heart. Most importantly, you are committed to delivering exceptional patient-centred care. To thrive in this role, you will have: Current and Unrestricted AHPRA registration. Minimum of 5 years previous experience in palliative care with a comprehensive understanding of the concepts of palliative care. Excelling communications skills with the ability to communicate with a diverse multidisciplinary team. Strong attention to detail and critical thinking skills. Current drivers licence You will also be required to provide evidence of, or in the process of obtaining: A current Police Record Check and Working with Children Check.
