We are seeking a dedicated NeuroCare Nurse to join our team and provide vital support services in the community, MSWA-supported clinic settings, and high-supported accommodation sites. This is a night-shift (7:00pm-7:00am), permanent, full-time role with travel required across the Perth metropolitan area. You’ll manage a diverse caseload of clients, both in their homes, our residential facilities and within the broader community, tailoring your support to each unique story and journey. What would the role look like? As a NeuroCare Nurse, you may start your evening by attending a multidisciplinary team meeting to discuss care plans and collaborate with health professionals. You then conduct a training session for MSWA staff on neurological care. You may journey to an MSWA-supported clinic, providing assessments and advisory services. You educate patients and families on managing symptoms and administering drug therapies. Later, you visit a high-supported accommodation site to offer specialised nursing care, such as continence and wound management, or facilitate referrals to other health services, ensuring a comprehensive care approach. Throughout the night, you visit clients in the community, providing palliative care support and collecting data for service statistics. You also work on NDIS funding applications and maintain client confidentiality. Your shift wraps up with a review of activities, planning for upcoming visits, and participating in a research project aimed at improving outcomes for people living with neurological conditions. You leave knowing you've made a significant impact on the lives of those you care for. To thrive in this role, you’ll need: Current registration with the Nurses Board of Western Australia as RN/EN. AHPRA registration for RN/EN. Current First Aid and CPR Certificate NDIS Worker Screening Check (or willingness to obtain). Full C Class driving licence without restrictions. Working with children check. Our 'nice to haves' Experience in Nursing individuals with Neurological Conditions and / or Community and / or rehabilitation nursing. Experience in working with people with neurological conditions. Knowledge of principles and objectives of Disability Services Legislation and an understanding of the National Disability Insurance Scheme. Experience in palliative care, continence management and wound management. Demonstrated excellence in oral and written communication skills as well as computer literacy. Demonstrated sound problem-solving skills and quality improvement practices. And the benefits?
