Location: Macquarie Park Job Type: Full-time ( 6 month contract) About the Role As a Care Coordinator at Anglicare, you’ll play a role in supporting older Australians to live independently in their own homes. You will support the coordinat ion of client services, ensuring they are aligned with client i ndividual's goals, wellbeing, and preferences. Working closely with internal teams, clients, families and community networks, you’ll be ensuring high-quality, person-centred care. Key Responsibilities Quality & Compliance – Ensure services are delivered in accordance with the Aged Care Quality Standards, Charter of Aged Care Rights, and relevant program manuals (e.g., HCP, CHSP). Documentation & Reporting – Maintain accurate and timely records, case notes, and client assessments in our digital systems. Stakeholder Engagement – Build strong relationships with clients, families, carers, and internal teams to support holistic care planning and delivery. Team Support & Feedback Management – Support the care team by capturing and logging client and service feedback through the RaiseIt system, contributing to continuous improvement. Problem Solving – Respond to changes in client needs, escalate issues appropriately, and ensure timely service adjustments. Continuous Improvement – Contribute to service enhancements, quality initiatives, and collaborative team discussions that improve client experience and outcomes.
