The team works collaboratively with participants, their families, and a wide range of service providers to deliver high-quality, person-centred support coordination services across regional Victoria. About the Role – Support Coordinator (Full Time) We are seeking an experienced and dedicated Support Coordinator to join our growing Support Coordination team on a full-time basis. This role is based in Ballarat or Bendigo, with service delivery extending across regional and metropolitan areas of Victoria. The ideal candidate will have: At least 2 years of experience as a Support Coordinator, or Extensive experience in complex case management, with a sound knowledge of the NDIS As a Support Coordinator, you will play a vital role in empowering NDIS participants to navigate their NDIS plans effectively and work towards achieving their goals. You will be responsible for coordinating and implementing the supports outlined in a participant’s plan, ensuring these are tailored to their unique needs while promoting independence, choice, and control. Key responsibilities include: Building collaborative relationships with participants, their families, and support networks Connecting participants with suitable service providers and community resources Monitoring service delivery to ensure it aligns with participants’ goals and preferences Advocating for participants’ rights and best interests Maintaining accurate and up-to-date documentation in line with NDIS Practice Standards This is a rewarding opportunity for someone passionate about making a positive difference in people’s lives, with the autonomy to work independently while being supported by a experienced and values-driven team.
