SCHADS 4B ($43.08 - $46.35 ph) Super Salary Packaging benefits Full time opportunity based in Ipswich/Toowoomba QLD About the opportunity This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through a mobile outreach hub. The Outreach Carer Services Navigator role is mobile and involves regular travel across the Ipswich/Toowoomba QLD region, providing consistent presence and reliable support to carers. Working with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to embed carer support locally. The role has three core functions: 1. Community Development: Using your community development, relationship building skills, establish and nurture collaborative partnerships with local organisations, building their capacity to identify, support, and refer carers. This includes ongoing training and support for community and frontline staff, embedding referral pathways, and ensuring the Mobile Carer Hub becomes a trusted, integrated part of the local service landscape. 2. Community Engagement: Conduct targeted outreach activities, campaigns, and events to raise awareness of Carer Gateway services and connect directly with carers who may not otherwise access support. This includes planned community events (e.g., Carers Week, NAIDOC Week) and informal engagements designed to identify and reach hidden carers. 3. Direct Carer Support: Provide accessible advice and guidance to carers, conduct intake and planning conversations, prepare individualised carer support plans, coordinate referrals to relevant services and facilitate opportunities for carers to engage in peer support activities.
