If so, we're looking for Carer Hub Navigators –who will be the welcoming face and guiding hand for carers in our new Carer Hubs. This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through in our Carer Gateway Hub. The Carer Services Navigator role is based in the Upper Mount Gravatt region, providing consistent presence and reliable support to carers however adopts a 'no wrong door' approach. Working in collaboration with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to further strengthen and embed carer support locally.
