Providing assessments, treatment, and ongoing support. Focus on educating clients about their diagnoses, treatment plan, lifestyle changes, medication and technologies. Implementing and coordinating care plans. Working with the multi disciplinary team, communicating with families, providing referrals to other services, organising follow ups.
