The Role: You will be working as a Clinical Nurse on the Continuing Care Team, following up with clients that have recently been discharged from hospital or acute services and require community care. Providing assessments, treatment, education and ongoing support. Administering and monitoring medications prescribed to your clients. Implementing and coordinating care plans Working with the multi disciplinary team, communicating with families and key workers, providing referrals to other services, organising follow ups.
