About The Role We are looking for a Wellbeing and Health In-reach Nurse (WHIN) Coordinator CNS2 to provide high-quality service to identified local schools and apply their specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school aged children and young people in these schools. The WHIN Coordinator CNS2 is a Registered Nurse who works in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school aged children and young people in these schools. Key Responsibilities Work with local school principals, school wellbeing teams, school counsellors and psychologists, school staff and local services to deliver the WHIN coordinator model of care serviceBuild trusting relationships with students and families to identify health needs and address barriers to accessing services Establish a family focused approach to connecting students and families to servicesIdentify and triage the health needs of students and families Assist in coordination of appropriate early intervention, assessments, and referral to services and programs Contribute to care coordination and case management of vulnerable children, young people and familiesSupport access to health promotion programs and services to improve the health and wellbeing of the school community and provide support for students and families to improve health literacy and access to servicesWork within locally agreed protocols and governance arrangements across Health and Education sectorsBuild and maintain relationships with service providers and establish effective referral pathways Collect and report on service events as required, maintain confidentiality and accurate health care documentation Find out more about this role, review the position description here.
