As Care Manager, you will be responsible for overseeing clinical assessments, care planning, and risk management to ensure the delivery of high-quality, person-centered care. Working alongside the Facility Manager and another Care Manager, you’ll play a key role in fostering a positive team culture, building strong relationships with residents, families, and staff, managing clinical budgets, and driving continuous improvement across the service.
